Please make check or money order payable to MidValley Soccer.

Registration Fees
Full Season: $90.00 which includes uniform, pictures and award.

Half Season (Fall 2009 or Spring 2010 only): $55.00 which includes uniform, pictures and award.

Fee Discounts (For Full Year Registration Only)
Families with multiple children registered receive a $5 per child discount.

Volunteers (coaches) receive a $15 discount (at time of registration).

Volunteers (board members) receive a $80/$90 discount ($15 at time of registration and the remainder at the end of the Spring Season).

Registration Information
To register, please dowload the registration form. You may register by submitting 2 completed copies (with original signatures) of the registration form and one copy of the volunteer disclosure form (if you wish to be a coach or other league volunteer), along with the appropriate registration fee, to:
